My cat's litter box has a strong ammonia smell

My cat's litter box has a strong ammonia smell, but they both are playful, have no issues of going outside the litter box. They have a water fountain, wet food, and dry. My only other concern is my older cat looks skinny, but the last time I took her to the vet he said she hadn't lost any weight. Could there be something wrong?

When I clean the litter boxes, I use a large paint scraper to scrape anything that's stuck to the bottom of the box, and dispose of it. I don't just skim over the top. I also add a little fresh litter to the litter in the box. As for the weight issue, the only way to find out if your old girl has any health issues is to observe her. Have her eating habits changed? Her behavior? If so, then you need to have a talk with your vet about this. He may want to perform blood tests and other checks to make sure she's OK. My Delilah also looks skinny to me, but the vet says she's fine, and actually her behavior hasn't changed at all. But if you are concerned, a chat with your vet about it is the way to go.

The smell is normal. I would dump out the old litter, clean the box really well and put fresh back in.
Keep the box very clean. Cats don't like stinky boxes and might find somewhere else to go.
