This really sounds like a flea bite. Behind the ear is a good place for them because the fur isn't as long and it's thin. Check your cat all over for fleas (you'll see them moving) and for flea dirt (poop) that looks like black pepper. If it turns out he does have fleas, ask your vet for prescription flea meds. Some of the flea meds sold in pet stores can be poisonous and the cheap ones don't work anyway. You'll also have to have his poop tested for worms. Both fleas and worms or parasites can also affect humans, so the sooner you do this, the better.
