Suite 103, Buzzer #60 17151 - 94A. Avenue Edmonton, AB T5T 5Z9 I am a single, handicapped senior with a well trained and socialized Chihuahua. She rides in the basket of my scooter when she is tired or we are going into a building.
I have trouble going outdoors and she helps me to do that and she breaks the ice for me with people who I am uncomfortable around.
I live in Edmonton, Alberta, Canada and I am trying to find who to contact to get a service dog vest and sign for this. I already have permission in writing from Mr. Rick Dimock who is Head of Environmental Health Services so I can continue to go into grocery stores to do my shopping. He has letters from my G.P. and Psychiatrist. I personally would like to have something visable on my scooter basket so people will know she is a service dog and not be bothering the store staff to complain. My dog does not bark and has spent the last 8 years going into the same grocery store with no accidents or problems. But I would like to know where on the internet for my city I would be able to contact to get these identification items. Thank you
Non of the categories below are what I need so I am just checking something off in order to get this to you.
