Your cat has feline hepatic lipidosis(FHL). I had this and my problem was related to my common bile duct being blocked. This a potentially fatal disease if you don't take care of your cat properly.
Yes you need to continue the antibiotics as a secondary infection could be part of the problem, and yes you need to continue the pepcid. There are other medicines that could be beneficial: Denamarin and Ursodial for liver support. You should discuss this with your vet. Do NOT stop any medications OR START any medications without your vet's specific approval.
You may want to consider seeing an internal specialist.
And you may want to consider a feeding tube if syringe feeding isn't working out. It is VERY important to keep up the caloric intake!
You may also want to Google Feline Triaditis and FHL. This information might help you to understand what you are dealing with.
You can P-mail me if you have any questions.
Good luck!

Thanks All. Here is what we are doing for Moo:

-2 antibiotics twice daily
- 1/4 pepcid twice daily
- subcutaneous fluids...100-150mls daily..this is the hardest part for me
- syringe feeding AD mixed with kitten milk...40-50mls daily
- 1/2 tsp Alpha Omega LiverTone daily
- lots and lots of LOVE

Moo is still pretty yellow looking especially around the ears, nose and mouth. He willing accepts the food by syringe but is still refusing to eat on his own. I bought Temptations as I read that cats love these but as with all the different brands of food I brought home...now Moo won't even eat the treats.

Doing the fluids is probably the hardest for us..takes three of us and my daughters are only 11 and 12.

Moo's stools have been consistent...a little yellow with brown mixed in but no diareha . He remains very social and sleeps with one of us each night.

Does anyone know how long thats? what other treats are tasty that I could give Moo.

I think he is recovering slowly..
thanks for all your help
Lisa and Moo

Here is a "liver shake" recipie for sick cats that works smile A vet came up with this recipie. Her contact info is below but I don't know how current it is. Your vet can contact this vet and ask about the recipie before you try it.

THE '~LIVER SHAKE~' FOR SICK CATS

This liquid food is appropriate for cats suffering from conditions where they are eating very little or not at all such as anemias, leukemias, deficiency states with symptoms of lethargy, listlessness, and weakness.

1 cup RAW LIVER (BEEF, or CHICKEN)
1 cup FRESH CARROT JUICE or V-8, or Tomatoe Juice
1/4 cup *FRESH FILTERED WATER
1 RAW EGG YOLK
1 tsp.KELP POWDER OR SPIRULINA

COMBINE AND MIX IN A BLENDER UNTIL LIQUEFIED. (Note: it will be frothy)

Dosage:

Administer 10cc daily 3-6 times a day. (6Occ per day max)

Some cats will need to be force-fed with a syringe, while others will drink it out of a bowl.

This food has a sweet and salty taste, and the energetic properties of tonifying (energy boosting), warming and stimulating and supports the kidneys, spleen, liver and stomach.

This drink is a complete dinner for a sick cat until they feel good enough to eat by themselves again.

*Substitute rice water if the cat has diarrhea. Rice water
is made by boiling white rice (2 Tbsp) with water (1cup) for 30 minutes.
This liquid aids digestion and assimilation and inhibits the symptoms of diarrhea.

Bump,
My internal specialist preferred Denamarin over denosyl (which we origionally stated on). She said the components in Denamarin provided better liver support.
I think I would be hesitant to introduce raw foods into this cat's diet right now (liver and egg) simply because the cat has been on antibiotics and raw foods could possibly introduce harmful bacteria (salmonella, etc).
Purrcy was inappetent, so he had a feeding tube. I just kept offering different foods to try to entice him to eat on his own. Sometimes he would eat a little and I would think - "AHA!! I found a food he likes!" and then he would turn up his nose the next time I tried that food.It can be very discouraging. Having a feeding tube also made giving fluids easier. I just syringed in 30cc a few times a day in between feedings. Cats associate their sickness with the foods they have been eating and often develop food aversion. That's why you need to keep giving the pepcid.
Glad Moo seems better. It takes a long time for the jaundice to go away. Wishing you the best!

When Smokey had her liver issues, ALT was 917, they ran it three seperate times, and double checked it each time, normal is 10-100. She was put on about every med out there, changed the dosages, changed the antibiotics, changed it from liquid to pills, back again, and she wasn't responding. One of the specalists who was working on her, said to try Denosyl. That one worked. She started responding right away, ALT started to come down. For her, it worked. They just had to find the right combo of meds, that would work for her, each cat is different. That was 3 months and $800, of meds and tests, to find what would work for her. waveway to gohug
