Cats can suffer from senility just like humans can. Or, she may be having vision problems. If the box is in the same place, you haven't changed litter and you are keeping the box very clean for her, then your first trip should be with her to the vet to make sure she has a clean bill of health. She may have a physical concern that's interferring with her usual good potty manners. You can ask the vet then how to help her relearn to use her box.
