I am 8 years old now. I stopped urinating in the litter box just recently. Nothing has changed and I'm spayed.
I urinate on the couch, on the floor, on curtains, and on clothes that are on the floor. My litter box is cleaned very often and nothing has changed at all. I've used the same litter for at least 3 years. The vet ran tests and assured my problem was behavioral and put me on medication to ease my stress. I still soiled the house when I was medicated.

What kind of tests did the vet do?
Whatever the case may be changeing your cats diet to an all wet diet that is high in protein and grain free is the best thing you can do.
Please read a site called catinfo.org there is lots of good info there.
You also need to buy a cleaner that is sold at petstores that will rid of the smell of cat urine. Natures Miracle is the best one, it nuetralizes the urine, doesnt just cover it up. Cats will tend to urinate where they have been before, so this is why you need the cleaner.

Is this the same vet that you all ways had ? It sounds like you may have a urine infection . I would try another vet if you can not get anywhere with this vet. Most times when cats urinate outside the litter box is because they had pain going and they feel that the litter box is what caused it.
