In unneutered adult male cats usually have pretty obvious testicles visible just below their tail. When cats are neutered the sac may still be there but there is nothing inside and it is just empty skin. Sometimes the testicles don't descend and are still inside the cats body. To neuter a cat like this the vet has to do an operation more like a spay for a female cat. If he appears to be neutered but still has really strong smelling pee and wants to roam and you've already had him for several months he may never have been fixed.

If you are not sure or don't really want to feel around take him to a vet and they'll be able to check for you and can arrange to get him neutered if he still needs it.
